Care Coordinators will be responsible for supporting a caseload of families with children and youth with intensive special health needs (CYSHN), from initial relationship development and assessment through development and implementation of individual service plans for each family and work to engage parents/ caregivers in peer support, learning, and leadership development activities. Care Coordinators will also be responsible for appropriately documenting their work with families and participating fully in the ongoing development of the Care Coordination team. Successful applicants will be motivated individuals who can work independently and must be detail oriented.
This position will work 37.50 hours per week.
Duties and Responsibilities (these are a general summary and not all-inclusive):
Conducts client and family assessments to identify service needs, facilitates the development and of comprehensive and developmentally appropriate individual service plans for children and youth with special health needs (CYSHN), and provides high quality, family-centered care, referrals, parent education and support directly to families of CYSHN, communities and providers.
Provides leadership for improving systems of care for CYSHN and their families consistent with the Medical Home Model.
Facilitates outreach and training to parents, community-based providers and educators.
Supports family engagement by developing and identifying culturally and linguistically competent leadership and training opportunities.
For transition age clients (14-22) develops a comprehensive plan for health care and life transition according to program standards and protocols.
Keeps records of all performed work in the CC database as well as other forms of documentation.
Participates in outreach, training, meetings and additional administrative and/or program activities as assigned by the Director of the Community Support Unit and the Director of the Division for CYSHN.
Consults with state agencies, service providers, parents’ organizations, private vendors and clinical professionals to coordinate service delivery; participates in multidisciplinary and/or multi-agency teams as required.
Participates in supervisory, team and other meetings as required.
Preferred Qualifications:
Experience working with computers and Windows-based applications.
Ability to work both independently and with others under time pressures and deadlines.
Ability to communicate effectively orally and in writing.
Willingness to adhere to strict confidentiality protocols.
Access to transportation for work related travel.
Bachelor’s Degree in social work, psychology, human services or related field plus minimum of 1 year experience in the field.
Bilingual skills preferred in one of the following languages: Haitian Creole, Spanish Vietnamese, Arabic.
Contract Period of Service: 6/6/2021 – 6/30/2022 – Renewable subject to appropriation
Additional Information:
Benefits are unavailable with the exception of sick leave accruals.
Contracts are renewable, subject to the Department approval, performance and the allocation of funds.
