The Opportunity:
Working within our Health Promotion Department, and an integrated service delivery team, the Concurrent Disorder Case Manager will provide support to youth and adults presenting with concurrent substance use concerns and mental health issues.

Key Responsibilities:
Provides trauma-informed counselling and integrated support services to a caseload of clients experiencing concurrent disorders;

Establishes a working and supportive relationship with clients and their support networks;

Assess clients’ mental, physical and emotional functional capacity and needs and investigates client’s present resources;

Supports assessment of clients and provides case management and support transitions to primary care and other community based providers as needed;

Devises a service plan with the client, establishes goals to meet determined needs, while providing direct services (counselling, groups, accompaniments, etc.);

Advocates, liaises, and coordinates service access with multiple community partners to support integrated service delivery;

Provides clinical navigation supports to support clients;

Advocates for client rights as necessary and supports self-advocacy;

Supports and assists family members with issues related to the client’s concurrent disorder as appropriate;

Provides skill teaching, solutions-focused counselling, and support to clients to assist in goal achievement;

Monitors symptomology, medication, and activities of daily living to help improve individuals’ ability to function on a day to day basis and within the community;

Provides crisis support response, as well as crisis prevention planning;

Provides community outreach services in health care settings, shelters, ERs, etc.;

Facilitates psychoeducation groups for clients and families;

Facilitates workshops and training with service providers to support concurrent disorder knowledge and skill acquisition;

Participates in community networking and advisory committees as appropriate;

Provides referrals and connections for clients to support long-term recovery and resiliency.

Mandatory Qualifications:
Bachelor’s degree in an Allied Health Discipline (Counselling Psychology, Psychiatric Nursing, Social Work, etc.) from a recognized program;

A minimum of 2-years of recent relevant experience working with dually diagnosed clients or a similar population, or an equivalent combination of education, training and experience;

Fluency in one of the contract languages (Arabic, Mandarin or Punjabi).

Qualified candidates will also posses:
Excellent communication and interpersonal skills;

Has personal lived experience with mental illness or substance misuse;

Strong computer skills, including Microsoft suite (Outlook, Word, Excel, and Teams);

Demonstrated ability to work collaboratively and independently;

Class 5 BC Driver’s License and access to a vehicle.

A criminal record check is required as a part of this process.
